Part II: Research, Teaching, and Clinical Contributions

 

A. Narrative Report

I am a full-time gastrointestinal and hepatopancreaticobiliary pathologist at the MGH. My research focuses on Barrett’s esophagus-related neoplasms and gastric neoplasia, with an emphasis on precursor lesions and early cancer. After authoring several articles on diagnostic and biologic aspects of dysplastic lesions of the upper gastrointestinal tract, I pioneered a field of study focusing on the pathologic assessment of new diagnostic and therapeutic modalities, i.e., photodynamic therapy (PDT) and endoscopic mucosal resection (EMR). My group was the first to recognize the importance of several features predicting response to PDT, and ongoing work is investigating differences in molecular alterations (aneuploidy, COX-2 expression) in an attempt to understand the variable response to treatment. We were also the first to complete several studies on the diagnostic and staging utility of EMR and to present guidelines for its evaluation. I also have been collaborating on evaluating the diagnostic capabilities of optical coherent tomography (OCT) in the screening and surveillance in patients with Barrett’s esophagus. In recognition of my expertise in the field, I have been asked to edit the next AFIP Fascicle on Tumors of the Esophagus and Stomach, to be published in 2009.

A secondary major research interest is pancreatic neoplasia and its mimics, notably autoimmune pancreatitis. With regard to autoimmune pancreatitis, my group published the first comprehensive analysis of the fine needle aspiration biopsy features of this entity and also has investigated the role of IgG4 in this disease. Notably, we have characterized the multicentric nature of this disease and are interrogating the role of T-cells in this inflammatory pathology. We were also the first group to show that the expression of keratin 19, a marker of ductal phenotype, is a powerful predictor of malignancy in pancreatic endocrine tumors. To date, our finding has been confirmed by five independent international studies and is rapidly being accepted as a new diagnostic marker. Finally, using well-characterized cohorts of pancreatic mucinous cystic neoplasms, we are currently developing definitive guidelines for the cytopathologic evaluation of these neoplasms. As recognition of my expertise in the field, I was asked to participate to the 2002 and 2007 American Joint Committee on Cancer hepatopancreaticobiliary working groups.

I am extensively involved in the education of pathology residents, GI pathology fellows, and clinical gastroenterology fellows at Massachusetts General Hospital. Since 2002, I have created and directed the MGH Gastrointestinal Pathology Fellowship. This one-year program provides in-depth diagnostic and clinical research expertise in gastrointestinal and hepatopancreaticobiliary pathology. Currently, all our graduates are in academic practice all over the country.  I have also mentored several international trainees who now hold academic positions worldwide. In addition, I serve as the director of Surgical Pathology Fellowship for the MGH. At the postgraduate level, I co-direct the “Gastrointestinal, Liver, and Pancreatic Pathology” course given each year in Boston. Finally, I am an Adjunct Professor of Pathology at Saitama Medical University, Japan, where I teach second-year medical students every year. I have given lectures nationally and internationally on various aspects of gastrointestinal pathology, and have organized and co-directed two international courses on GI pathology. I have served on program and abstract committees for a number of national and international pathology meetings, am on the editorial boards of four leading clinical pathology journals, and serve as an Associate Editor-in-Chief of the Archives of Pathology and Laboratory Medicine.

My clinical work centers on gastrointestinal pathology, including directing the gastrointestinal pathology service at the MGH and GI pathology consult cases from pathologists around North America and overseas. My administrative duties involve the directorship of the Gastrointestinal Pathology Service. I have forged a strong diagnostic group, now one of the best in the nation. I am proud of the scientific mentorship provided to my younger colleagues, two of whom have recently been promoted at HMS and are emerging as national figures in the field of gastrointestinal pathology. I also have been instrumental in fostering strong collaborative clinical, investigational, and educational efforts with the Divisions of Gastroenterology and General Surgery. Since October of 2006, I have also been Co-Director of Surgical Pathology, the largest faculty division of the Pathology Service at MGH. I have enabled a number of significant changes in this division, using new information systems and laboratory technology while enhancing patient safety, staff satisfaction and quality of clinical and translational research.  Finally, I also direct the MGH Tissue Repository (”tumor bank”), which provides tissue throughout the Partners system and beyond. Under my leadership, the bank has been able to offer new services and to recruit several previously independent banks into our standardized, IRB-approved system, which has directly benefited the MGH scientific community.

E. Report of Clinical Activities

 

Since July 2000, I have been Chief of the Gastrointestinal Pathology Service in the Department of Pathology at the MGH. This unit diagnoses about 14,000 cases per year. The clinical load encompasses the gamut of benign and oncologic material.
